Open a new document in photoshop, 350 x 19. Any color background.

At the bottom of the layers palette, click the second button from the left to add a layer style. Choose gradient overlay.

Click the black and white bar to edit the gradient. The gradient editor window should open.

The larger gradient at the bottom in black in white has 4 boxes. The top two are Opacity Stops and the bottom two are Color Stops. Double click on each color stop and choose the colors you wish to use.

When you get back to the layer style window with the gradient overlay, you can move the gradient just by moving your mouse over the bar and dragging the bar up and down.

Next, you'll need to create a new layer and apply a scanline pattern.

After you apply the pattern, drop the opacity of it.

Now you need to find an image of what you want to put on the bar. I find it easy to find a picture with a solid or no background. Open the image and press Ctrl + A to select all, then Ctrl + C to copy, finally Ctrl + V to paste it on the bar.

If you image is too big, you will have to scale it down.
Go to Edit > Transform > Scale. Hold Shift, Ctrl, Alt, and click and drag one of the corner points to the center of the image.

You can also go to Edit > Transform > Rotate, to rotate it to your liking.

Now add the text with a small pixel font. Make the text white and add a layer style to it like you did the first layer, except this time, select stroke. Change the size to 1 and the color to Black.

I guess this is the hard part

Create a new layer and select the Elliptical Marquee tool.

If there is a rectangle there, just right click it and select the circle.

You may want to maximize the bar window for this. The glowing button.

Just like any other window.

Position you mouse above and to the left of the bar. Now click and drag your mouse until it makes the same shape as below.

Make sure you are on the new, blank layer. Press D, then press Ctrl + Delete/Backspace. It should be filled with white. Now lower the opacity of that layer just like the scanlines.

That's basically it. If you want to add a border, here you go.

Make a new layer over all the others and press Ctrl + A. Go to Edit > Stroke.
Size = 1
Color = Your Choice.

Suggestion:
If you have PS, PSP, or whatever, I suggest you put as many as you want into one thing. Just save it as a transparent gif. As many bars as you want into one file. That will keep you from going over the 500 character limit while trying to adjust your signature.
